An SEO edit is easier to judge when the reasoning fits on one card. Capture the observation, the proposed change, the expected reader benefit, and the date when the result can reasonably be reviewed.
Separate observation from explanation
“The page receives impressions but few visits” is an observation. “The title is the cause” is a hypothesis. Keeping those statements separate prevents a plausible story from becoming a fact.
Make the change narrow
Prefer one interpretable edit, such as aligning a description with the page’s actual answer. Avoid changing the title, structure, links, and call to action together unless the page is being deliberately rebuilt.
Define the stopping point
Name when the page will be reviewed and what evidence would support keeping, revising, or reversing the edit. Search performance is affected by many variables, so the card should never promise attribution that the available data cannot establish.
Pair the card with an SEO change observation window. Together they make routine improvements reviewable without pretending that every movement has a single cause.
